Karen Levy Associates Ltd has a legal duty to explain how we use any personal information we collect about you, as a registered patient at the practice. Staff at this practice maintain records about your health and the treatment you receive in electronic and paper format.

What information do we collect about you?
We will collect non-sensitive information such as personal details, including name, address, a record of appointments, e-mail address and telephone number. We require this information contractually in order to provide our service in accordance with article 6(1)(b) of the GDPR-EU legislation. We may also collect sensitive data when it is relevant to enable us to deliver effective therapeutic care. This is in accordance with article 9(2)(h) of the GDPR-EU legislation.

We may also hold information about you from your GP, Psychiatrist or another practitioner who might write to us on your behalf.

How we will use your information
Your data is collected for the purpose of providing direct patient care; however, we can disclose this information if it is required by law, if you give consent or if it is justified in the public interest. We may share information about you with your GP or with your psychiatrist if we believe this would be beneficial to your health.

Maintaining confidentiality and accessing your records
We are committed to maintaining confidentiality. Your data is stored securely and is protected. We will retain your records for a period of 5 years after treatment has ended.

You have a right to access the information we hold about you. Furthermore, should you identify any inaccuracies in that information, you have a right to have the inaccuracies corrected. You also have the right to have your personal data restricted or deleted if you feel it is being used improperly, unless it has to be retained by law.
